We are delighted to offer an online session on complex discharge pathways with Ashleigh Fox of Catalyst Care Group. 

Ashleigh Fox is a leading advocate for transforming care for autistic people and people with a learning disability. As the whistleblower at Winterbourne View in 2010, Ashleigh’s actions ignited the national Transforming Care agenda. Now, as the Transforming Care Director at Catalyst Care Group, Ashleigh works to create real, lasting change by developing innovative, person-centred support models that help individuals transition from hospital to community living. 

Session Overview: 

This session will explore the evolution of bridging support, from the lessons learned at Winterbourne View to the pioneering transition model now implemented by Catalyst Care Group and the LD Network. Ashleigh will share how proactive, bespoke support can prevent unnecessary hospital admissions and expedite safe discharges - helping individuals thrive in their own communities. 

This talk aims to offer valuable insights into navigating complex discharge pathways, reducing restrictions, and ensuring people receive the right support at the right time. 

Attendees will gain a deeper understanding of the Bridging Support Model, and how collaborative working and innovative support models are driving better outcomes for people at risk of being institutionalised.
